Rally in the Valley @ Phila Oaks is limited to a 9 Ball 32 Dream Team Event and 8 Ball 160 Dream Team Event. 1st Come, 1st Serve. No Entries will be accepted after MONDAY, MARCH 14 th or filled up at 160 teams whichever comes first. All Checks should be submitted, with this form, made payable to TAP, LLC and sent to P.O. Box 368, Dauphin, PA 17018. Any Questions, please email Kelly Nace, kelly@tapleague.com PLAYERS MUST HAVE A CURRENT TAP MEMBERSHIP CARD WITH THEM, OR THEY WILL NOT BE ALLOWED TO PLAY. ID CAN BE VERIFIED BY THE CAPTAINS FOR THE TEAMS THEY ARE PLAYING IN COMPETITION. Fee for Dream Team Event is $550.00 per team. Each Team Name must be its own. 20 Characters or Less. No Team Names 1 and 2. Registrations are NON Refundable, but they are transferable up to the deadline of March 16th. Receipt of Form and Payment by the deadline is the only guarantee of your spot, unless all spots have been filled at time of receipt. Must have played 6 matches from Nov. 23 rd to March 14th In weekly league play. Players must meet all national requirements, see rulebook. NOTE: Each Player can only be on two (2) Rally Dream Teams. In addition two (2) HC 7’s are allowed on one team however can play one match per scoresheet, not both same round.ng stations serving a variety of food and beer on tap.
